Setting Up Custom Fields in HighLevel Forms
Step 1: Navigate to Forms
To begin, navigate to the “Sites” section in HighLevel and head over to “Forms.” Here, you can select any of your existing forms or create a new one. For this example, we’ll work with an existing form.
Step 2: Access Custom Fields
Previously, when you clicked plus and then clicked “Custom Fields” within forms or surveys, you wouldn’t have seen the new UI. This update allows you to sort your custom fields by contact-related fields or opportunity-related custom fields.
Creating Opportunity-Specific Custom Fields
Step 3: Add a Custom Field
Now, let’s create a custom field to further qualify the leads filling out your form. This will help you get higher-quality leads within your pipeline. Click “Add,” and a familiar section will appear. Create a single-line custom field to ask for relevant business information, like the current revenue of the lead’s business.
Step 4: Set Field Properties
Give your field a title, select the object as “Opportunity,” and choose “Opportunity Details” as the group. Hit save, and the new field will appear under “Custom Fields Opportunity.” Now, you can add this field to your form.
Automating Lead Qualification
Step 5: Configure Opportunity Settings
When you click on the new field, you’ll see a popup asking where you want your opportunity to be updated. This feature allows you to run automations directly within the form. Select your primary pipeline and the stage where you want to place the new lead. For this example, we’ll use the “Send Video Sales Letter” (SVSL) stage and set the opportunity value accordingly.
Step 6: Save and Implement
After configuring the settings, hit save. You can always edit these settings later by clicking on the field and selecting “Edit Opportunity Settings.” Once saved, the form is ready to capture and manage lead information automatically.

Let’s see how this works in action. Here’s a pipeline with no contacts in the SVSL stage. Preview the form, fill it out, and submit it. Since we configured an opportunity-based custom field, the contact should automatically appear in the SVSL stage of your pipeline.
Step 8: Verify in Pipeline
Refresh your pipeline to see the new contact in the designated stage. Clicking on the contact within your opportunity will show the details, including the source of the opportunity (form) and the custom field information filled out by the customer.
